Hepatic Resection for Hepatocellular Carcinoma -Retrospective study of 49 cases -


Abstract
The hepatic resections were carried out in 49 patient with hepatocellular carcinoma who had admitted in Kyung Hee University Hospital during the 7-year period from 1987 to 1994.
The result were as follows; The male: female ratio was 6 : 1 and average age was 52.6 year. Thirty severn patients had liver cirrhosis and HBs Ag positive rate was 75.7% in the cirhotic patients compared with 58.3% in the noncirrhotic patients.
The
associated disease were 35 cases of liver cirrhosis, 9 chronic active hepatitis, 7 clonorchis sinensis infestation and 2 cases of biliary sone. The 28 patients had tumor size smaller than 5 cm in diameter. The Child classification of the patient
was as
follow; 34 cases (69.4%) of Grade A, 11 cases(22.4%) of Grade B and 4 cases(8.1%) of Grade C respectively. the operative mortality was 8.2%. The postoperative course was complicated by pleural effusion (32.6%), atelectasis(26.5%), hepatic
failure(12.2%), fever of unknown origin(41.%), intra-abdominal hemorrhag(4.1%) and wound infection(4.1%). The overall 1,3 and 5 year survival rate was 75.3%, 51.8% and 28% respectively. the volume of oeprative blood loss was the most decisive
factor
affecting the short term prognsis. Prognostic factor that influenced the logn term prognosis were investigated by comparing the survival curve. The alpha-fetoprotein. Tumor size, capsular invasion and vasculr invasion were showed significant
difference
of survival patterns(P<0.05). The rcurrence of hepatocellular carcinoma was the main cause of death in 17 cases(74%) who died later than 1 month.
